I had 410cc implants 2 yrs ago as I wanted my breasts to be firmer and shaped instead of sagging. I am looking at options as to whether I should remove them completely or reduce them with an uplift. Can I reduce them without an uplift? A reduction with uplift would be better but I don't want excess…

The answer to your question will surely depend on the amount of breast tissue that is left and the size you are looking for after the procedure. For sure whether you downsize the implants or remove them totally, you will end up with some sort of sagging and will need a lift. 34 ff size is quite large and has its own set of problems. By having the breast lift you can still have your breast firm and perky without the extra size your are carrying. Please consult with an ASAPS member well versed in those procedures. Is Restylane use safe in the tear troughs? I have been considering this procedure for some time for chronic dark circles under my eyes but I know it is considered an "off label" use of Restylane. Is it really safe and effective?

Restylane is relatively safe. However, in some cases it might cause a blueish discoloration due to it being monophasic. This is called tendal effect and it tends to happen in a lesser percentage of biphasic HA products. I'm 30 years old I have one pendulous breast with the nipple directed outward and downward. I want to undergo minimal lift of this breast to look similar to the other breast. Is the crescent incision better than donut incision? Can I undergo this surgery with local anesthesia?

Several techniques could be used to correct your case ranging from the crescent incision to the donut incision or even sometimes a circumvertical incision. The one that is best for you depends on how pendulous your breast is and how elastic your skin is and whether there will be a need to decrease the areola or not as compared to the contralateral sides. I'm 65 and had breast reduction surgery 46 years ago. My areolas were removed and a vertical incision was made to the natural fold. No" fold" incision was made. Although my breast size was reduced, there was no lift, which I desperately needed. Now, more so. Is it possible to reduce my breasts again…

Yes, in principle you can have a breast reduction the second time. A breast lift is always a part of the breast reduction. Having the operative report from your first surgery will defensively help your surgeon determine which technique is best for you to provide you with the wanted results in the safest way.
