Am I a candidate for breast reduction? I have very large breast (40n) with grooves in my shoulders. I suffer from back, shoulder and neck pain. I recently lost 24 lbs but the breast are lot to carry around. I’ve been to the doctor on and off about pain but no relief. I buy larger shirts so they want…

You describe signs and symptoms that are similar to those of tens of thousands of women who each year undergo reduction mammaplasty (breast reduction). Large breasts, disproportionate for your body, shoulder grooving, and back, neck & shoulder pain, Your recent weight loss should improve your general health and help your plastic surgeon achieve the best lifted, reduced breast appearance for you. You will join many women who shop for more flattering and comfortable clothes, and participate in an active lifestyle. Reduction can do wonders for your outlook on life.

I have had saggy breasts since I was young. About fifteen years ago I had a breast lift. The surgeon made the nipples too high. It was obvious very quickly as my nipples popped out of all bras and bathing suits. Then as the years went by, my breasts began to sag again yet my nipples stayed high so…

The procedure you describe is an accepted method to revise and re-tighten the lower breast after a prior breast lift, and an excellent way to improve the relative nipple position (lower). It is not as common as other breast lift techniques, which may have something to do with fewer surgeons mentioning it. It is good that you have discussed your interests and concerns with various surgeons, so that you can select one that meets your goals and expectations.

Is it likely that insurance would cover a breast reduction? I'm 5'4, 42F, and 229 lbs. I've been wanting one for years but I don’t think I have any significant physical problems that would consider it 'medically necessary', but my new insurance looks like it would cover it if I removed enough grams…

Yes. Most insurance companies will cover reduction if the patient has shoulder, upper back and neck pain, cup size DD or larger, and if a substantial amount of breast tissue is estimated to be removed. The estimated removal would be determined by your plastic surgeon during your in-person consultation. Your situation raises another question: timing of surgery vs. weight loss. Breast reduction would make it easier for you to be active, which should help you lose weight. The weigh loss may make the breast more loose or relaxed. On the other hand, your plastic surgeon could give you the best breast if you lost excess weight prior to the reduction surgery. Discuss this with your surgeon.

You would not be wasting your time having consultation with experienced board certified plastic surgeons. He or she can address your concerns, evaluate your breasts, abdomen, flanks, etc. - areas which will be evaluated for "mommy make-over." In-person evaluation is essential to determining whether the surgeon can make appropriate changes and meet your expectations. You have, very reasonably, waited years since your bowel obstruction. Your plastic surgeon may want to consult with your general surgeon for his or her assessment about your abdominal contents and abdominal wall (possible hernias, etc.). In the end you should be able to enjoy the benefits of your major weight loss.
