Breast Reduction

My current size is a 30-I, and I would like to get to a D or a DD if possible. My question pertains to how much tissue can safely be taken out?

You can achieve your desires with a reduction with the guarantee being smaller. Achieving a specific cup size is more challenging and tissue removal is generally safe. BUT nipple issues doe arise the farther and farther the nipple has to be elevated but this can be minimized by using specific techniques or even nipple grafts in extreme situations. So ask your surgeon what kind of pedicle is being used and what strategies will be employed to minimize nipple loss.

The situation you describe should easily be managed with an inferior pole plication (side to side scar above the fold) as the simplest way to manage your concerns. More durable outcomes can be achieved with removal of the hanging tissue and replacement with an implant (if you're okay with that) and using an inferior based de-epithelialized flap t help protect your implant (aka reduction augmentation) or trying to reposition your tissues so you have more fullness on top, assuming that your prior procedure did not burn a lot of bridges in terms of moving tissue safely.

Nipples can be made firmer, thicker, and maybe a little longer with fat grafting or dermal filling (transient). If you really want significant length, some kind of reconstructive procedure will be needed involving transfer of tissue/grafting to your existing nipple. Perhaps stretching using 'barbells' could be done to lengthen it and then fill with fat as a less extreme procedure. Best to see a local board certified plastic surgeon for an exam and if the above is not mentioned, you could bring it up to their attention and thoughts and criticisms.

One breast larger after a year

Hello! I had my breast augmentation in November 2017. Everything was good until yesterday, when I noticed that one of my breasts is larger than the other. No pain or other symptoms, just larger. Maybe it's somehow connected with my menstruation which came very early this month!?

When a year out and you have an obvious change in breast size that doesn't resolve on its own over a few days from first appreciating it requires that you see your surgeon for an exam and for answers to what may be occurring. If the size difference is progressive, even more urgent to see your surgeon.
